Vaginoplasty Techniques: Types and Differences

There are several gender-affirming vaginoplasty techniques, and the main difference between them lies in the tissue used to construct the vagina and whether or not a vaginal canal is created. The three most common are penile inversion, colovaginoplasty, and zero-depth vulvoplasty. The appropriate technique is not the same for everyone: it depends on each person’s anatomy, medical history, and goals.

In this article you will see what each technique involves, what advantages and limitations it has, for whom it is indicated, and how they differ from one another. If you are not yet clear about the procedure in general, you can start by understanding what vaginoplasty is and then return to this comparison.

Penile Inversion: The Reference Technique

Penile inversion is the most widely used technique and the standard in genital affirmation surgery. It involves using the skin of the penis and scrotum to construct the vaginal canal, labia, and vulva, while part of the glans is used to create a clitoris that retains sensitivity, as its nerve bundles are preserved.

Its great advantage is that it uses the person’s own tissue, which offers a natural, functional result with good erogenous sensitivity. It is the indicated option for most cases. It requires subsequent dilations to maintain the depth of the canal, an aspect that forms an essential part of vaginoplasty postoperative care.

Colovaginoplasty: When More Depth Is Needed

Colovaginoplasty uses a segment of intestine (usually colon) to create the vaginal canal. It is especially indicated in cases where penile inversion cannot provide sufficient depth, for example when there is little available tissue or in revision surgeries.

Among its characteristics is that intestinal tissue generates lubrication naturally. As it involves abdominal surgery in addition to genital surgery, it is a more complex technique and is reserved for specific indications that are assessed on an individual basis. Like any technique with a vaginal canal, it also requires specific follow-up and postoperative care.

Zero-Depth Vulvoplasty: Without Vaginal Canal

Zero-depth vulvoplasty constructs the external genitalia (vulva, clitoris, and labia) with feminine appearance and sensitivity, but without creating an internal vaginal canal. It is an option designed for those who do not desire vaginal penetration, prefer to avoid dilations, or seek a less complex surgery.

Its main advantage is precisely that: as there is no vaginal canal, postoperative dilations are not necessary, which simplifies recovery. It is a personal and legitimate decision that more and more people are consciously choosing, and which is assessed in consultation as a complete alternative, not as a second-choice option.

Other Techniques in the Current Landscape

In addition to the three above, in the field of genital affirmation surgery other techniques are practiced such as peritoneal vaginoplasty, which uses the peritoneum (the membrane lining the abdominal cavity) to line the vaginal canal, often combined with penile inversion. Not all surgical teams work with all techniques: each unit specializes in those it masters and with which it obtains the best results. At Gender Medic the reference techniques are penile inversion, colovaginoplasty, and zero-depth vulvoplasty.

How Is the Appropriate Technique Chosen?

The choice is not determined by a trend or a generic preference, but by an individual assessment. The consultation takes into account the anatomy and available tissue, medical history and previous surgeries, whether or not the person desires a vaginal canal with penetration, willingness to perform postoperative dilations, and each person’s aesthetic and functional goals.

That is why two people may receive recommendations for different techniques, both being correct: what works for one person does not necessarily have to be ideal for another. Prior hormone therapy is not mandatory, although it is recommended, and factors such as tobacco, alcohol, or obesity influence the outcome with any technique.
